After three rounds of clubs, you might discard a spade on the third round, in case West started with a doubleton club. If you ruff with the heart 8 instead, West (holding a doubleton club) might overruff with the queen of hearts, and you might later lose a spade finesse. But these are a lot of mights. There was a clue that the clubs are not divided 2-6. East would have overtaken the 10 with his jack at trick one if he held six of them, in case the 10 was a singleton.

The best plan is to ruff the third club with the 8 of hearts (answer to question A). If this holds, next lead a heart to the ace (answer to question B). As the cards lie, you next finesse hearts and make four. But if West has three hearts to the queen, you can lead a second heart to the king and play the ace of diamonds, a diamond to the king and ruff a diamond. Then lead a third heart. West will win the queen and be endplayed, forced to lead a spade or give you a ruff-sluff.
